American Cancer Society - Patient Navigation Volunteer

Project Description:
The Volunteer Patient Navigator will establish contact with cancer patients within the hospital or cancer treatment center setting through approaches agreed upon with the host site to raise awareness about American Cancer Society information resources, support programs and services, community resources, and assist in meeting other cancer-related needs articulated by patients, families, and caregivers. The Volunteer Patient Navigator will establish an ongoing relationship with the health care community and the host location of the ACS Patient Navigator Program.
